Who may Avail?
1. Filipino farmers and fisherfolks;
      2. Micro, small and medium scale enterprises engaged in agriculture, aquaculture and fisheries sector;
      3. Agriculture and fishery cooperative and other entrepreneurs engaged in economic enterprises related agriculture and fisheries;
      4. Local Government Units (LGU) and other implementing bureaus and attached agencies and corporations under DA’s
      5. State Universities and Colleges.

Service: ISSUANCE OF CERTIFICATE OF ELIGIBILITY PER EO 376
Schedule of Availability of Service:
      Monday to Friday, 8:00am to 5:00pm
Who may avail of the Service:
     All Enterprises engaged in agriculture and fisheries provided that the imported agriculture and fishery inputs, equipment and machinery shall be for the exclusive use of the importing enterprise.
What are the requirements:
      A. Small Agriculture & Fisheries Enterprise:
            1. Duly accomplished application form
            2. A certified copy of certificate of registration issued by:
                     i. The SEC, in the case of corporation and partnership;
                     ii. The DTI, in the case of single proprietorship; or a certified copy of Business/Mayor’s Permit, 
                     as the case maybe;
                     iii. The CDA, in the case of cooperative; or
                   iv. By the SEC or an accreditation by appropriate government agency in the case of farmer’s/ fisherfolk’s
                     organizations and associations; and
            3. Declared assets of the previous year or the latest available declaration of assets, which will be the basis of 
                     the total allowable peso value of import transaction/s in a year.
      B. Agriculture & Fisheries Enterprise with a total assets of more than 15m pesos:
            1. Duly accomplished application form;
            2. A certified copy of the certificate of registration issued by:
                     i. The SEC, in the case of corporation and partnership;
                     ii. The DTI, in the case of single proprietorship;
                     iii. The CDA, in the case of cooperative; or
                   iv. By the SEC or an accreditation by appropriate government agency in the case of farmer’s/fisherfolk’s 
                      organizations and associations;
            3. Duly “audited” assets of the previous year or the latest available declaration of assets, which will be the basis of the total allowable peso value of import transaction/s in a year;
             4. Notarized statement indicating the following:
                     i. That the enterprise has no pending application from other issuing agency/ies under the incentives provided in the Section 109 of RA 8435 as amended by RA 9281; and
                     ii. That the enterprise has not availed of similar incentives provided under existing law for the same articles to be imported; and
             5. An indicative list of regular used inputs, equipments and machinery for the year.
